I am currently serving in Hyderabad, but have been transferred to Bangalore. My family will be staying back in Hyderabad due to children's education. My queries:

1. Can I retain the same CGHS card for myself and family. I will be frequently visiting Hyderabad and wish to get treatment in Hyderabad. (This is my preferred option)

2. If not, can I have two CGHS cards, one for myself at Bangalore and the other for my family at Hyderabad. In such a case who will be issuing me the card for my family, my present office or the future office at Bangalore. Will the CGHS in Hyderabad accept the card issued by my office in Bangalore.

3. In case the CGHS refers for a test/specialised procedure or admission in a hospital, from where should I/my family members get the referral form signed. Since I would be at Bangalore, it would be impractical for me to get it signed for every test, procedure, admission etc.

Multiple CGHS cards

You can have two CGHS cards one for your family and another for yourself. Recent we have issued two cards for one of our staff who is in chenna and his dependants are in Bangalore. See Nabhi's CGHS compiation book.
