Is there any certificate required if the wife of a govt. servant hospitalised for cesarean delivery at any other place (his home town) other than his place of duty.

If it is govt hospital no other certificate is required(.) however in case of private hospital emergency certificate is mandatory

Sir Hospital is covered under CGHS. In that case certificate is required or not. If required then what should be written in the certificate.

pratibhaji, from your reply it seems it is a pvt. hospital on cghs panel. In that case emergency certificate is must. And mostly the format is readily availaible with them.
